No monthly fee: the practice always receives the full amount, and Belmont Labs earns only a published service fee on the patient's side.

Video visits

Built for a doctor who sees twenty patients a day, not a demo.

Before the visit

  • Schedule from the patient's chart: date, length, visit type, reason, which physician. The patient is notified at once and reminded the day before and 30 minutes ahead.
  • Patients complete intake in the app (symptoms, medications, allergies, temperature and weight if they have them) so the visit starts with the facts.
  • The patient tells the app what state they're in; visits are allowed only where the physician is licensed.

During and after

  • The dashboard counts down to each visit and turns red when the patient is waiting. Tap the row to open the room. The physician gets a push 10 minutes ahead.
  • 1080p video and echo-cancelled audio, encrypted end to end between the two devices. Live quality readout on the physician's screen.
  • Complete the visit with CPT and ICD-10 codes; the copay charges the card on file and a draft claim is created for the biller.

Prescriptions

Sent electronically to the pharmacy the patient chose, the way Texas requires.

The patient's pharmacy is already on the chart

Patients pick their pharmacy in the app by searching the federal NPI Registry ("HEB, 78006"), so the record is the real store with its phone, fax, and NPI. It shows on the chart with call and copy-fax buttons.

E-prescribing through Surescripts

With a DoseSpot account connected, the physician prescribes from the chart and the script goes to the patient's pharmacy over the Surescripts network, including controlled substances with EPCS. We set the account up with the practice; until it's connected, the Prescribe button stays hidden rather than pretending.

Remote patient monitoring

Readings from an assigned, FDA-cleared cuff, counted the way the codes require.

For the care team

  • Assign a cuff to a patient from the chart (make, model, serial). Readings pair to that device.
  • Out-of-range readings alert staff; each review is logged as monitoring time with one tap.
  • Patients who go quiet for three days are listed on the dashboard with a call button.
  • Blood-pressure report for any date range: averages, ranges, trend, flagged readings; export as PDF or CSV.

For the biller

  • Month view per patient: days with readings, staff minutes, whether there was live contact.
  • Suggested codes from the counts: 99453 setup, 99454 (16+ days) or 99445 (2–15 days) device supply, 99457 and 99458 management, 99470 for shorter months.
  • Export the month as a spreadsheet for whoever files the claims.

Billing and payments

Copays, balances, financing, and insurance, without the practice touching a card number.

Patient payments

  • Cards on file, entered by the patient directly into Stripe. Copays charge automatically after each visit; failed cards are listed for the front desk with the reason.
  • Balances after insurance appear in the patient's Wallet; they pay by card or split it with Affirm. Every fee is itemized before they confirm.
  • Each in-app payment produces a 9-digit redeem number (with a QR). The front desk enters or scans it, the funds transfer to the practice's Stripe account, and the balance is marked paid.
  • Earnings screen: pending release, released this month, Stripe balance and next payout.

Insurance claims

  • Insurance captured in the app with card photos; verify-coverage flag on the chart.
  • Completing a visit creates a draft claim (CPT, ICD-10, place of service 10, modifier 95).
  • Choose how the practice bills: export for the biller, through the practice's EHR, through a clearinghouse (Claim.MD direct; Office Ally and Availity by 837P upload), or self-pay only.
  • Claims queue for the biller: price from the fee schedule, send, record the outcome. Patient responsibility flows straight into the patient's Wallet.

Fees on the patient's side: 2.9% Belmont Labs service fee and 2.9% + $0.30 processing on card payments; 6% and 6% + $0.30 on Affirm. Published at primarycarelive.com/#fees. The practice receives the full balance or copay.

Messaging, staff, and security

Secure messaging

  • Patients write to the office by category (scheduling, billing, medication, results). Attach photos, PDFs, Word or Pages files up to 50 MB.
  • Staff inbox with assignment and resolve; every message stays in the app, never in email.
  • The practice sets its hours and reply promise; patients see them.

Staff and permissions

  • Invite each person with an 8-character code and a role: physician, clinical staff, front desk, biller.
  • Per-person permissions for charting, billing, redeeming payments, prescribing, and managing staff. Physicians enter NPI and license on joining.

Getting started

Register

Open the app, choose "I'm a doctor or practice staff," and register the practice. The owner can be the physician or the office manager.

Set up payouts

Stripe verifies the practice and links the bank account. About five minutes; the app walks through it.

Invite the team

Generate a code for each person. Physicians enter their NPI and license when they join.

Give patients the code

Your practice code goes on a card or your website. Patients create their account with it and land in your practice.

E-prescribing requires a DoseSpot account, which we set up with the practice. Direct claim submission requires the practice's own Claim.MD account; the other billing options need nothing extra.
